Service of Process/Proof of Service Form

After a process server or PI serves legal papers, they must then fill out a form which confirms that the papers were served. This form goes by many names: Proof of Service, Affidavit of Service, Certificate of service, etc. Sometimes the client provides the blank form, other times the server must come up with the blank form. Usually, the server can find the proper form on a court website; the court in which the case was filed. The server then fills out the form with the information about the serve; when, where and to whom, and then either sends the form to the court, or sends the form to the client, who then files it with the court. Under some circumstances, the form must be notarized, and the server has to visit a notary.
